Payment Bundle Resources

Information on payments made to the facility for defined bundles of services and procedures is available at https://pricing.floridahealthfinder.gov. The service bundle information is a non-personalized estimate of costs that may be incurred by the patient for anticipated services, and actual costs will be based on services actually provided to the patient. The patient can request a personalized estimate from the Center at any time before or after the procedure is performed. Patients should also contact the health care practitioners anticipated to provide services to the patient while in the center regarding a personalized estimate, billing practices and participation with patient’s insurance provider or HMO as the practitioners may not participate with the same insurers or HMO as the center.

Estimates and Information

Seascape Surgery Center (SSC) is committed to providing meaningful information to our patients related to financial obligations for healthcare services. The best way for patients to determine their out-of-pocket costs in advance of their visit is to contact their insurance company. However, a patient may request an estimate of anticipated charges for non-emergency care from the Center.

In order for the SSC to provide prospective patients with a good faith estimate, your physician’s office must have already scheduled your surgery or procedure with the center. Upon request and as required by Florida Law, SSC will provide to prospective patients an estimate of gross charges, before any adjustment for your healthcare plans or self-pay discounts in writing within seven (7) business days. Financial Assistance

Please contact our facility’s billing office for further information on our policies at 813-977-2040. Surgeons, anesthesiologists, and other service providers (such as pathologists or laboratories) bill for their services separately from the surgery center and may offer their own financial assistance program—please contact them for further information regarding their services.

Charity care

Seascape Surgery Center does not currently have a charity care policy.

Collection procedures

As a courtesy to our patients, we will file an insurance claim on behalf of the patient to his/her insurance plan. A patient is expected to respond to his/her insurance plan’s request for information timely, as needed, to minimize processing delays with the claim.

Patients are expected to pay their financial obligations in a timely manner including the estimated portion by the day services are received, and any remaining portion upon finalization of the claim by the payer. Unpaid claims by the payer may result in the account’s outstanding balance being fully transferred to the patient for collection.

If needed, the center will attempt to reach a patient by any method available to us to secure payment on the outstanding balance utilizing internal and external resources. If the account becomes delinquent, it may be placed with an attorney or agency for collection in which their fees and expenses may be the obligation of the patient.

This facility submits specimens to different pathology providers so please inquire with the facility as to which pathology provider will be used. Some pathology providers may engage specialized pathologists to assist in examining the patient’s specimen.

The attending physician who booked your procedure(s) at this facility, and other health care providers not limited to but for example anesthesiologists, intraoperative monitoring services, durable medical suppliers, and anatomic pathologists may provide services to you at the facility that are not a part of the facility’s fees. If any of their services are provided to you while being a patient of this facility, the provider(s) may or may not bill separately for their rendered services.

A personalized estimate of charges, including but not limited to specific contact information prior to service, can be provided upon request.

Patients should contact each health care provider who will provide service(s) related to their procedure(s) to determine health care network or preferred provider status.
